These resources are starting points. A template becomes more useful when it is adapted to the actual recipient, issue and jurisdiction.

Replace all placeholders with accurate information.
Confirm that you are contacting the authority or organisation actually responsible for the issue.
Check current legislation before making legal claims.
Support health, statistical and research claims with appropriate evidence.
Distinguish personal experience from evidence about a wider population.
Do not describe an acknowledgement, referral or meeting as an endorsement.
Keep the request within the recipient's actual powers and responsibilities.
Keep a copy of the correspondence and any response received.
